You take a couple of weeks off running, stretch your hips, and spend some much needed quality time with your foam roller. Your knee finally feels better, so you decide to lace up your shoes and go for a jog to get back on your training plan. After a few short minutes that familiar ache on the outside of your knee starts to creep back up and before you know it the pain is sharp and excruciating again!

Sound familiar? One reason IT band pain can keep returning is that **feeling better during rest doesn’t necessarily mean you’re ready for the demands of running.** Time off often helps symptoms settle down, but jumping straight back into your usual mileage can bring the irritation right back because you didn't address why it started in the first place

The IT band is a thick strip of connective tissue that runs along the outside of your thigh, connecting the hip and knee. When this area becomes painful, it also feels tight, so it’s easy to assume it just needs more stretching. But when you have recurring symptoms like this, it deserves a broader look than just stretching. How quickly did you increase your mileage? Did you recently add hills or speed work? Do your hip and leg muscles have the strength and endurance to support repeated single-leg loading that running requires? Training demands, strength, and running mechanics can all be relevant, and the combination looks different for each runner.

These are ALL things we help Columbus runners manage on a daily basis.

Breaking the cycle means building a bridge between pain relief and your normal training. Depending on your assessment, that may include hands on soft tissue work, treatments such as dry needling, progressive hip and leg strengthening, exercises that challenge single-leg control, and adjustments to your running routine. Your return to running might begin with short run-walk intervals on flat ground, with recovery days between sessions before gradually rebuilding distance and adding more demanding workouts. The most important thing is to pay attention to how your knee responds not just during the run, but afterward too. If pain builds more with every minute, changes your stride, or leaves you more sore the next day, your progression and plan may need adjusting. The goal is to build repeatable tolerance rather than test how much you can push through on a daily basis.

If every return to running ends with the same frustrating knee pain, it’s time to understand what’s keeping that cycle going. At Competitive Edge Physical Therapy and Performance, we help active adults in the Columbus area assess their symptoms, training history, strength, and movement to build an individualized plan. We also make sure pain on the outside of your knee is actually consistent with IT band irritation before treating it that way. (Because it may be something else entirely!)
