# The 1st Step to Heal Sciatica Pain

One of the most basic ways of identifying if our treatments for sciatica are working relies on the concepts of centralization and peripheralization. 

 

Centralization➡️ symptoms move closer to the source, this is what we want to happen!

Peripheralization➡️ move further away from the source, what we DON'T want

In this case, the source of symptoms is the irritation at the nerve root, or the area where compression is happening. These symptoms are often pain, numbness, tightness and can be more intense and go further down the leg with peripheralization.

 

When assessing sciatica, how much something hurts is only part of the picture. **Where you feel your symptoms, and how that location changes with movement can provide valuable information about your response to an exercise.**

The pain doesn’t have to physically “travel” upward to centralize. You may simply notice that it no longer reaches as far down your leg.

Research has associated centralization with better outcomes in some people with sciatica. It gives us a useful clinical clue about how you’re responding to movement.

Easy... right??

For someone that has never heard of these terms before, or don't know what they're looking for, it can be downright confusing. But the most effective treatments I have ever used rely heavily on the centralization and peripheralization principles described above.  If we can use your bodies natural movements and positions to reduce the pressure on the nerve, then we can help the pain and disability go away much quicker and get you back to the activities you love!

Applying the principles of centralization and peripheralization into your rehab and daily life will be the key to getting a full and fast recovery, and it's much easier than you think!

**Is Your Sciatica Actually Getting Better? Pay Attention to Where It Hurts**

Imagine you’ve been dealing with an annoying ache in your buttock that sometimes shoots down the back of your thigh. Sitting bothers it, your workouts feel different, and you started trying exercises at home, hoping something will help.

After one exercise, the pain drops from a 4 out of 10 to a 2. Finally, some progress!

But there’s something else... the discomfort now reaches the back of your knee.

Does that still count as improvement?

The intensity improved, but the symptoms spread farther from your back. That change deserves attention, especially if it persists or happens repeatedly with the same movement.

It doesn’t automatically mean you damaged something. It does mean that the lower pain score alone isn’t enough to decide whether the exercise is a good fit.

Now imagine a different response. Before an exercise, you feel discomfort halfway down your thigh. Afterward, that thigh discomfort disappears, but the ache around your buttock becomes more noticeable.

Your first thought might be, “This made me worse.”

Possibly...but there’s more to assess. The retreat of the leg symptoms may be favorable, even though discomfort closer to your back increased. The next questions are how intense that discomfort is, whether it settles, and whether your overall ability to move improves.

**This is not a reason to push through escalating pain. It’s a reason to assess more than the pain score.**

If your symptoms keep changing or you’re unsure which movements are helping, an individualized evaluation can put those observations into context and help you build a plan for getting back to lifting, running, and moving comfortably!
